We did this as a web survey, and for the audience we selected, we got an ~50% response rate. But, this was a carefully selected audience of junior faculty, K scholars, people we thought would be more motivated to respond (and more likely to be interested in CTSI - so not random at all).

....minor question - I tried to "forward to colleague" and it said "access denied." Is this a future feature, or was I doing something wrong?

How was this survey distributed? Was it a Word form, a paper form, and/or a web form? What was your response return rate?
